Open the app's Settings and tap Restore purchases. Make sure you're signed in to the same Apple Account you bought it with. If it still doesn't appear, email us with the date of purchase and we'll look into it.
Yes — everything is included in your device backup. Restore the new phone from that backup and your data comes with it. There is no account and no cloud sync, so a fresh install on a new device starts empty.
